The radiological manifestations of the initial phases of rheumatic carditis in childhood

Abstract

One hundred and eighty six cases of acute rheumatic fever during the first attack were reviewed. These cases were children in the age groups from 4 to 14, hospitalized in the ‘Rambam’ Hospital, Haifa, in the period 1956–1962 inclusive. Complete x-ray examinations with contrast visualization of oesophagus at the time of admission were available in every case and also in most cases before discharge.There was clinical evidence of active carditis in 68 patients and the radiological manifestations of the initial phases of active rheumatic carditis were studied in this group. In 54%, the heart was of normal size and shape, yet 25% of these developed rheumatic valvular disease, providing that a heart of normal radiological appearance does not exclude the presence of initial active rheumatic carditis.It was shown that in cases presenting generalized enlargement of the heart silhouette of varying degrees (stage 1–3), making up 46% of the cases, the radiological appearances follow closely those seen in acute myocarditis. No radiological signs of a valvular lesion can be expected in these early stages of the disease.The changes in size and shape of the heart were demonstrated radiologically, discussed and compared with other forms of acute myocarditis. The significance of the distension of the upper lobe veins was analyzed and evaluated with attention paid to the time of its occurrence in the initial phases of the disease.
